Organizations face many unfortunate events where the common reaction is to blame certain managers for making major mistakes. This common behavior does not help in preventing such mistakes to happen in the future.  Other events might seem to be too good to be true, which actually means there is a lot to learn from such surprising success.  These events contain huge opportunities to improve the way the organization functions.  These events also signal the possible emergence of a threat to the organization.  Such internal learning is especially crucial in times of change where many older paradigms have to be updated with the new reality.  The culture of blaming is replaced by the culture of learning from experience where the notion is: "we are doing mistakes – BUT we do them only ONCE, we do learn how to do it right next time.  ” This is an integral part of "The Learning Organization” idea.
